Information to include

• Your name and a contact address.
• A description of the original work and where it can be found.
• The specific EasyInvestCalc URL or image you are reporting.
• An explanation of your ownership or authority to act and why you believe the use is unauthorised.

For a formal statutory notice, include the statements and signature required by the applicable law. Seek legal advice if you are unsure of those requirements.
